Rule 5: . Issue of duplicate Registration Certificate :-
If the Certificate of Registration issued under rule 3(3) is lost, destroyed or defaced, the employer of the establishment shall forthwith report the matter to the Inspector of the area concerned and shall apply in Form-VI with a fee as prescribed in Schedule-II for the issue of duplicate registration certificate. Upon the receipt of such application together with the fee, the Inspector shall furnish to the employer with a duplicate
copy of registration certificate duly stamped Duplicate copy of the Registration Certificate.
