Form XXI - Register of Fines

This covers establishment as well as principal employer particulars, nature of work, location of work, employee details, act/omission for which fine is imposed, date of offence, whether opportunity to show cause is given, wage period, wage payable, amount of fine imposed, and date of realization.

Contractor shall maintain a register as per the given format, contract wise, principal employer wise, and establishment wise.
