Form D - Statement of Employers & Employees' Contribution

The employer's contribution and the employee's contribution in respect of a year shall be paid by the employer to the Karnataka Labour Welfare Board along with this statement on or before 15 January of the following year by cheque or crossed demand draft, and the expense of remitting the contribution shall be borne by the employer. 

In respect of every employee whose name stands in the register of an establishment on 31 December of a year, the sum of Rs.6, Rs.12, and Rs.6 shall be payable as contribution for that year by the concerned employee, the employer, and the State Government respectively.
