Form 20 - Form to claim the dues by Nominee

In the event of the death of a member, the nominee or the family member shall use the given Form to claim the member's Provident Fund dues.

For final settlement in favour of nominee/beneficiary of a deceased member, this Form will be used by:

• a nominee or legal heir of the deceased member.
• guardian of a minor/lunatic member.
• guardian of the minor/lunatic nominee or heir for claiming the Provident Fund accumulation of the minor deceased member.
