Form 20 - Claim for Maternity Benefit by a Nominee

On account of a deceased insured woman, if the death occurs after leaving the child behind, the nominee of the insured woman shall submit a claim in the given format.

This Claim Form, duly filled up, is required to be submitted to the appropriate branch office together with a death certificate within 30 days of the death of the insured woman.
