Employee has to fill the Form, affix two postcard size phtographs, and hand over to employer. This Form, duly filled, must reach the concerned Branch Office within 10 days of appointment of an Employee. Delay attracts penal action against employer under Section 85 of the Act.
Employee is supposed to intimate ESI Corporation on any changes in the membership of his family within 15 days of such change.
