Form U - Combined Annual Return

This is a mandatory filing requirement for any establishment and includes establishment details, employment particulars such as employees working and employees separated, leave with wage particulars, welfare measures, maternity benefits provided, and contract labour information.

The employer who submits Form U under Karnataka Shops and Establishments Act need not submit similar information under the following enactments:

1. Form XXV Rule 82(2) of Contract Labour (Regulation & Abolition) Karnataka Rules, 1974
2. Form III Rule 22(4) of Karnataka Minimum Wages Rules, 1958
3. Form XX Rule 20(1) of Karnataka Payment of Wages Rules, 1963
4. Form L Rule 16 of Karnataka Maternity Benefit Rules, 1963
