Rule 5: Issue of duplicate Registration Certificate:-
If the Certificate of Registration issued under sub-rule (3) of Rule 3 is lost, destroyed, or defaced, the employer of the Establishment shall forthwith report the matter to the Inspector of the area concerned and shall apply in Form-VI with a fee as prescribed in Schedule-II for the issue of duplicate registration certificate. Upon the receipt of such application together with the fee, the Inspector shall furnish to the employer with a duplicate copy of the registration certificate duly stamped ‘Duplicate copy of the Registration Certificate’.
