Form-D along with supporting documents.
Form-E: Acknowledgement of intimation.

Post-Registration Changes
Change in Registration: File Form-G within 30 days. Fee structure same as initial registration.
Closure of Business:
Form-H (10+ workers) / Form-I (<10 workers).
Inspector may remove entry suo moto if non-functional.
Employment of Women in Night Shifts
Women may work between 9:00 PM and 6:00 AM only: With their written consent in Form-J,
If employer ensures:
- Safe transport (with GPS & CCTV),
- Group deployment (≥3 women per shift),
- Internal Complaints Committee (under PoSH Act),
- Well-lit, secure premises.
Shifts and Work Arrangements
- Form-K: Weekly holiday notice.
- Form-L: Shift-wise deployment list (to be displayed + shared with Inspector).
- Minimum 12-hour rest between day/night shift changeovers.
- Part-time employment allowed up to 5 hours/day.
Worker Identification and Leave Management
- Form-M: Identity card for each worker.
- Form-N: Leave Book (earned leave, casual leave, festival leave).
- Form-O: Notice of leave accumulation limit (max 45 days).
Workplace Facilities and Welfare
- Cleanliness & Ventilation: Mandatory.
- Fire Safety: Must follow fire department guidelines.
- Latrines & Urinals: Gender-specific, with soap and water.
- Crèche: Required if women with children are employed.
- First Aid: Must include specified items (e.g., burn dressings, antiseptics).
- Canteen: Mandatory for 100+ workers with hygiene and pricing norms.
Registers and Returns
- Form-P: Combined Muster Roll cum Wage Register (manual or digital).
- Form-Q: Annual Return (due by 31 Jan).
- Records to be preserved for 5 years.
- All forms must be in English or local language.
Compliance, Inspection & Offences
Inspector's Powers:
- Randomized inspections,
- Serve notices and guide employers.
- Form-R: Application for compounding offences.
- Form-S: Declaration of managerial staff.
- Form-T: Declaration of confidential staff (max 1% of total, up to 50).
